“BACKGROUND: The treatment of complex diaphyseal malunions is challenging, requiring extensive preoperative planning and precise operative technique. We have developed a simpler method to treat some of these deformities.

METHODS: Ten patients with complex diaphyseal malunions (including four femoral and six tibial malunions) underwent a clamshell osteotomy. The indications for surgery included pain at adjacent joints and deformity. After surgical exposure, the malunited segment was transected perpendicular to the normal diaphysis proximally and distally. The transected

segment was again osteotomized along its long axis and was wedged open, similar to opening a clamshell. The proximal and distal segments of the CCI-779 PI3K/Akt/mTOR inhibitor diaphysis were then aligned with use of an intramedullary rod as an anatomic axis template and with use of the contralateral extremity as a length and rotation template. The patients were assessed clinically and radiographically at a mean of thirty-one months (range, six to fifty-two months) after the osteotomy.

RESULTS: Complete angular correction was achieved in each case; the amount of correction ranged from 20 to 20 in the coronal plane, from 0 to 32 in the sagittal plane, and from 0 to 25 in the axial plane (rotation). Correction of length ranged from 0 to 5 cm, and limb length was restored to within 2 cm in all patients. All osteotomy sites were healed clinically by six months. While no deep infections occurred, superficial wound dehiscence occurred in two patients along the approach for the longitudinal portion of the osteotomy, emphasizing the importance of careful soft-tissue handling and patient selection.

“Purpose

The purpose of this retrospective study was to identify factors predictive of survival in rectal cancer patients who

received surgery with curative intent after preoperative chemoradiotherapy (CRT).

Materials and Methods

Between July 1996 and June 2010, 104 patients underwent surgery for rectal cancer after preoperative CRT. The median dose of radiotherapy was 50.4 Gy (range, 43.2 to 54.4 Gy) for 6 weeks. Chemotherapy was a bolus injection of 5-fluorouracil and leucovorin for the first and last week of radiotherapy (n=84, 77.1%) or capecitabine administered daily during radiotherapy (n=17, 16.3%). Low anterior resection (n=86, 82.7%) or abdominoperineal resection (n=18, 17.3%) was performed at a median 47 days from the end of radiotherapy, and four cycles of adjuvant chemotherapy was administered. The serum carcinoembryonic

antigen (CEA) level was checked at initial diagnosis and just before surgery.

Results

After a median follow-up of 48 months (range, 9 to 174 months), 5-year disease free survival (DFS) was 74.5% and 5-year overall survival (OS) was 86.4%. Down staging of T diagnoses occurred in 32 patients (30.8%) and of N diagnoses in 40 patients (38.5%). “Leptospirosis is a spirochetal bacterial infection of great public health importance. It has a broad spectrum of clinical manifestations which goes from subclinical infection and self-limited anicteric febrile illness (80-90% of all cases) to icteric leptospiropirosis known as Weil’s disease. This is a severe disease characterized by hemorrhage, acute renal failure and jaundice. It is uncommon for leptospirosis to present itself as a primary neurological disease. Additionally, acute pancreatitis is an unusual gastrointestinal manifestation. We report a case of leptospirosis presenting as ascending progressive leg weakness and complicating with acute pancreatitis in an adult patient treated at Hospital Universitario, Universidade Federal de Santa Catarina. The diagnosis was confirmed through ELISA-IgM antibody testing positive for leptospirosis. After antibiotic therapy and support treatment for a few weeks, total resolution of severe manifestations was achieved.

“Arabinogalactan proteins (AGPs) are highly glycosylated members of the superfamily of hydroxyproline-rich glycoproteins (HRGPs). Despite their implication in many aspects of plant growth and development little is known about their role in tomato fruit ripening (Solanum lycopersicum) and their response to abiotic stress in tomato fruits. A search of the currently available tomato genome database resulted in the identification of 34 genes encoding putative AGPs, with at least 20 of them being expressed in fruit.

“BACKGROUND: The present work aims to gain further insight on the use of Fusarium solani pisi cutinase from a Saccharomyces cerevisiae strain for the synthesis of short chain ethyl esters, a group of important fruit flavor compounds, in a non-conventional

environment. Synthesis is promoted by cutinase in organic media, in particular, in iso-octane, an organic solvent recognized as a safe ingredient in food and beverage industrial processes.

RESULTS: The effect of solvent and substrate components CX-6258 nmr of the reaction mixture on the enzyme stability was measured separately. Focus was given to the effect of reaction medium on the operational stability of cutinase and on the esterification yield after 24h. The feasibility of the operation in fed batch mode was successfully evaluated. The effect of the addition

of substrate in consecutive pulses in the activity and stability of the biocatalyst was again assessed.

“Many neurons in the macaque ventral intraparietal area (VIP) are multimodal, i.e., they respond not only to visual but also to tactile,

auditory and vestibular stimulation. Anatomical studies have shown distinct projections between area VIP and a region of premotor cortex controlling head movements. A specific function of area VIP could be to guide movements in order to head for and/or to avoid objects in near extrapersonal space. This behavioral role would require a consistent representation of visual motion within 3-D space and enhanced activity for nearby motion signals. Accordingly, in Fer-1 concentration our present study we investigated whether neurons in area VIP are sensitive to moving visual stimuli containing depth signals from horizontal disparity. We recorded single unit activity from area VIP to two awake behaving monkeys (Macana Mulatta) fixating a central target on a projection screen. Sensitivity of neurons to horizontal disparity was assessed by presenting large field moving images (randon dot fields) steresoscopically to the two eyes by means of LCD Shutter goggles synchronized with the stimulus computer. During an individual trial, stimuli had one of seven different disparity values ranging from 3 degrees uncrossed- (far) to 3 degrees crossed- (near) disparity in 1 degrees steps.
